Preliminary results of synchronous acquired PET/MRI for viability assessment in patients with reduced cardiac function prior revascularization

چکیده
Methods We wanted to assess the interrater reliability of viability assessment in a hybrid PET/MR system, to assess if it could have additional value for patients prior revascularization. Therefore we examined 9 patients, scheduled for viability assessment by PET as a simultaneous PET/MR examination (mMR Biograph, Siemens, Erlangen, Germany). All patients were examined 50-60’after i.v. application of a mean activity of 319 MBq. The MRI protocol consisted of Cine SSFP sequences in 2 chamber, 4 chamber and short axis orientation (SA). For late enhancement assessment phase sensitive inversion recovery sequences (PSIR) in SA were acquired 10-15 minutes after administration of 0.15 mmol/kg body weight Gadobutrol (Gadovist, Bayer Healthcare, Berlin, Germany). 7 patients got 50g glucose and a individual dose of insulin, depending on blood glucose level. In 2 patients suffering from type 2 diabetes mellitus a standardized glucose clamp was performed. One patient got

Functional cardiac magnetic resonance imaging (MRI) in the assessment of myocardial viability and perfusion: an evidence-based analysis.
OBJECTIVE The objective of this health technology policy assessment was to determine the effectiveness safety and cost-effectiveness of using functional cardiac magnetic resonance imaging (MRI) for the assessment of myocardial viability and perfusion in patients with coronary artery disease and left ventricular dysfunction. RESULTS Functional MRI has become increasingly investigated as a noni...
متن کاملAssessment of myocardial viability by dobutamine echocardiography, positron emission tomography and thallium-201 SPECT: correlation with histopathology in explanted hearts.
OBJECTIVES We examined the relationship among viability assessment by dobutamine echocardiography (DE), positron emission tomography (PET) and thallium-201 single-photon emission computed tomography (TI-SPECT) to the degree of fibrosis. BACKGROUND DE, PET and TI-SPECT have been shown to be sensitive in identifying viability of asynergic myocardium. However, PET and TI-SPECT indicated viabilit...
متن کاملMyocardial viability in preserved or mildly impaired left ventricular function prior to revascularization - findings from a 3 year experience
Background Viability testing prior to revascularization in ischaemic cardiomyopathy has courted controversy in the literature over recent years. Viability assessment prior to revascularization at our institution is largely requested for patients with severely impaired left ventricles (LV). This study however aims to review the outcomes of the patients with preserved or mildly impaired LV functi...
متن کاملMyocardial perfusion reserve after a PET-driven revascularization procedure: a strong prognostic factor.
UNLABELLED Not all patients treated on the basis of PET-proven viability benefit from revascularization. Myocardial perfusion reserve (MPR) predicts survival in patients not undergoing revascularization. In the present study, we investigated whether MPR is related to survival in ischemic heart disease (IHD) patients after a PET-driven intervention. METHODS Between 1995 and 2003, 119 consecuti...
متن کاملClinical applications of positron emission tomography in cardiology: a review.
Myocardial viability assessment is of utmost importance in the assessment of patients with poor left ventricular function undergoing revascularisation therapies or cardiac transplantation. Cardiac positron emission tomography (PET) is widely regarded as a "gold" standard in myocardial viability assessment. We review the current data on this subject with respect to tracers and techniques, and pr...
